What are the wellness activities?
To help you better understand your health, you and your covered spouses or partners have the opportunity to complete three voluntary wellness activities:
- Health questionnaire
- Health screening
- Attestation that you have a primary care physician and have had an annual physical within the last 12 months
Your health screening includes height, weight, waist measurement, blood pressure and cholesterol, and your health questionnaire is a set of questions about your overall health, both completed on My Wellness (mywellnessresources.com). All three activities can help you get to know more about your health, and keep up to a $500 credit toward your annual medical plan premiums (and an additional $500 credit if your covered spouse or partner completes their three voluntary activities, as well).
Once you complete your health questionnaire on My Wellness and complete your health screening by Feb. 28, 2023, you’ll keep a $250 credit. If you also attest to having a primary care physician with an annual physical within the last 12 months by Feb. 28. 2023, you’ll keep the additional $250 credit as long as you have completed the health screening and health questionnaire.
Your covered spouse or partner can also keep a $250 wellness credit if they complete their health questionnaire on My Wellness and complete their health screening, by Feb. 28, 2023. If they attest to having a primary care physician with an annual physical within the last 12 months, they’ll keep the additional $250 credit. The additional $250 credit can only be kept if you complete both the health screening and health questionnaire by Feb. 28, 2023.
